Privacy Policy
Your privacy is important to the Velvet Blue Psychology. This privacy statement provides information about the confidentiality of your treatment with our practice and how we collect and use personal information about you.
Privacy and Record Keeping
Your personal information (obtained verbally, via hardcopy or via electronic forms) will always be kept secure in a locked filing cabinet or electronically secured and password-protected online. Information you provide will be used for the purpose of providing psychological services only.
Under the Privacy Act 1988 you are entitled to access your personal information at any time. Requests to access or correct your information should be made in writing and will be responded to within 30 days of receipt of request.
Your files will be kept for a minimum of seven years after your last client contact unless legal or organisational requirements specify otherwise. In the case of records collected while the client was less than 18 years old, we will retain records at least until the client attains the age of 25 years.
Confidentiality
Your personal information will be kept confidential and will not be shared without your written consent, except in the following situations:
There is a risk of harm to yourself,
There is a risk of harm to others (including children), and/or
Your client file is subpoenaed by a court of law.
In any case, you will be notified before your file is released.
With prior verbal and written consent your psychologist may wish to contact your treating health providers, e.g. GP, Psychiatrist, other allied healthcare providers.
Appointments Reminders
A reminder SMS / text message will be sent to you regarding your booked appointment and you will be asked to confirm your attendance.
